Physical Therapist – General opportunity in Bala Cynwyd, Pennsylvania. Starting 10/21/2026 for a 12-week travel assignment in a home healthcare setting. Responsibilities
- Deliver physical therapy services to home-based patients per the plan of care
- Conduct start-of-care visits and revisits for appropriate patient populations
- Assess functional status, gait, balance, range of motion, strength, and activity tolerance
- Develop and implement individualized treatment plans with measurable goals
- Document evaluations, daily progress notes, and periodic reassessments in patient records
- Communicate findings and coordinate care with the supervising PT and interdisciplinary team
- Provide patient and family education on exercises, safety, and home-modification recommendations
- Ensure patient safety during treatment and adapt interventions to the home environment

Requirements
- Active Physical Therapist license in Pennsylvania
- 2+ years of home health experience
- Start of care and revisit experience required
- Ability to work effectively in a home healthcare setting
- Willingness to travel within the defined ZIP code service areas and manage a home-based caseload
