Overview
Physical Therapist, General, Tillamook, OR. Start date: 11/16/2026. Travel assignment for 24 weeks. Weekly pay: $2,576–$2,736 per week. Guaranteed hours: 40 per week. Shift: 4x10 days, day shift (approximately 7:30 AM–5:30 PM). This travel opportunity is in a rural, critical access care setting.

Responsibilities
- Conduct initial evaluations and ongoing reassessments to inform individualized treatment plans.
- Deliver physical therapy interventions using evidence-based techniques and modalities.
- Collaborate with an interdisciplinary team to coordinate care and set patient-centered goals.
- Maintain accurate, timely clinical documentation including evaluations, progress notes, treatment plans, and discharge summaries.
- Supervise assigned assistants and delegate clinical tasks as appropriate.
- Provide patient and family education on treatment plans, safety, and discharge planning.

Requirements
- Doctor of Physical Therapy (DPT) degree required; licensure in the state of practice required; license eligibility rules apply if obtained after January 2016 or with foreign equivalent.
- Physical Therapy licensure in the state of practice (Oregon) required.
- CPR or Basic Life Support (BLS) certification from an approved vendor required.
- Two years of Physical Therapy experience preferred.
- Ability to work in a rural care setting and meet the 4x10 schedule requirements.
