Home Health Field Nurse (RN/LPN) – West Indianapolis, IN | Current Patient Needs

Elite Home Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for current home health patient needs in Southwest Indianapolis, Clayton, and surrounding Central Indiana communities .

As a Home Health Field Nurse, you'll provide one-on-one skilled nursing care in the patient's home , allowing you to focus on the individual needs of the patient while building meaningful relationships with the patients and families you serve.

• Southwest Indianapolis – RN needs
• Clayton – Skilled Nursing Needs

Patient assignments, schedules, available hours, and required nursing credentials vary based on individual patient needs and the Plan of Care.

Field Nurse Responsibilities

As a Home Health Field Nurse, you will provide skilled nursing services in accordance with the patient's Plan of Care and assigned nursing responsibilities.

Responsibilities include:

• Assessing and documenting the patient's condition
• Reviewing medications and vital signs
• Administering physician-prescribed medications as ordered and permitted
• Providing skilled nursing care according to the patient's Plan of Care
• Educating patients and families on home healthcare procedures and strategies
• Coordinating care with physicians and other members of the patient's healthcare team
• Communicating effectively with the interdisciplinary care team
• Completing accurate and timely clinical documentation
• Reporting relevant changes in patient condition
• Following patient-safety, infection-control, privacy, and Elite Home Healthcare procedures

Qualifications

Candidates must:

• Be a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current credentials authorizing practice in Indiana for the assigned position
• Demonstrate strong clinical nursing skills
• Have excellent written and verbal communication skills
• Be detail-oriented and organized
• Demonstrate effective time-management skills
• Be compassionate, dependable, and comfortable working independently in patients' homes
• Be able to understand and follow the patient's Plan of Care and applicable clinical orders
• Complete applicable Elite Home Healthcare employment, credentialing, screening, and compliance requirements

Home health, private duty nursing, community-based nursing, or related experience is valued.
